| An O-2 FAC (call sign Jake 4) was on its way to a mission when the aircraft collided with a UH-1D helicopter (66-01016) of the Army’s 174th Aviation Company near Quang Ngai. The Huey had just taken off from LZ Dottie and was manoeuvring to avoid machine gun fire when it collided with the O-2 at about 1,000 feet. Maj Brenner was killed when his aircraft crashed into a canal as were the four crew and three infantrymen on board the helicopter. One of the dead was Lt Col F A Barker, CO of the 4th Battalion, 3rd Infantry Regiment. |
