Date: 23 June 1968
Aircraft type: F-4D Phantom
Serial Number: 66-8724
Military Unit: 497 TFS, 8 TFW
Service: USAF
Home Base: Ubon
Name(s):
Lt Col Donald Francis Casey (KIA)
1Lt James Ervin Booth (KIA)

Lt Col Casey and 1Lt Booth were flying a night mission to attack a target in the mountains near Xom Sung, north of the Mu Gia Pass in North Vietnam. Just as their aircraft (call sign Casey) rolled in to drop its bombs, the wingman saw a flash and a large fire on the side of a mountain. Attempts to contact Lt Col Casey met with no success and it became obvious that the Phantom had crashed. Whether the aircraft had been shot down or had flown into high ground in the dark remains uncertain.
