| An O-2 FAC collided in mid-air in Quang Nam province with an Army CH-47C Chinook 67-18513 from C Company of the 159th ASHB, 101st Airborne Division. The five crew of the Chinook (consisting of pilot WO T L Dives, CWO R A Vaquera, Sgt D A Irelan, Sgt T K Ryan and SP4 J S Stacey) were also killed in the accident. |
