| An O-2 (call sign Jake) was flying under low cloud on a visual reconnaissance mission in the Quang Ngai region when it failed to return to its base at Da Nang. The aircraft is thought to have been shot down near the village of Ba To, about 20 miles south of Quang Ngai, and 1Lt Duffy was probably killed in the crash. His remains were returned to the USA in May 1993 and identity confirmed in March 1996. |
